You have good answers on the others. I disagree with the wording of all possible answers to #1. The least wrong is actually answer C.

A. Ozone is *made of* a single element, and that element is not ozone, it is oxygen

B. Ozone is made from oxygen, which is found in our atmosphere as diatomic molecules. However UV-C or more energetic radiation, shatters some of those molecules into individual atoms, and occasionally one of those loose atoms joins with an oxygen atom to make ozone. So really it is made from one diatomic molecule and one atom.

C. After the oxygen atom is shattered, it is moving very fast. It heats molecules it passes by, losing momentum, and finally temporarily sticks a nitrogen molecule (or argon). Not really forming a molecule, usually shown as N2O*, the oxygen is easily given up to any passing target, including an oxygen molecule (making ozone). So ozone is most commonly formed in both our atmosphere and in commercial ozone generators, with the help of... nitrogen. No nitrogen in the ozone molecule, it just acts like a catalyst to ozone formation.

D. Ozone is formed also by the interaction between violet or more energetic light, NOx (a combustion byproduct), and VOC (unburned fuel). But this does not help make the "protective layer that protects [us] from UV from the Sun". This is the ozone that is smog.
